2010-2011 Enrollment Procedures for CUHS

ENROLLMENT PROCEDURES

1. Contact Counselors 

              Mrs. Gwen Morgan   gmorgan@usd493.com or

              Mrs. Patty Hodgson   phodgson@usd493.com

              Or call them at   (620-429-3821)


a. Bring required information to expedite the process; counselors can obtain the  

       information, however, if you can bring it.


            i. Withdrawal grades
            ii. Transcript
            iii. Immunization Records- Students cannot be enrolled without these records
            
iv. IEP if available

2. Alternate Enrollments


a. Suspension: All suspensions are honored, so enrollment will not occur until the     suspension is completed.
b. Students unable to graduate with four years of high school attendance may be required to attend a CUHS alternative program.
c. No credits will be accepted for courses taken at a non-accredited school.

3. New Student Procedure


a. Student and parent fill out enrollment information sheet
b. Parent completes the request for records
c. Students will not be enrolled officially until:
            i. The information is received from the last school and
            ii. The students address is verified to be in the district

4. While the schedule is being created, the student and parent can be signing necessary forms.

5. Enrollment fees are $35.00

6. The next day the student will come to school.  He/She will need to pay the fees and report to Mr. Shearburn.  Mr. Shearburn will give them appropriate materials.
