NEW IMMUNIZATION REQUIREMENTS FOR 2009/2010 SCHOOL YEAR

Diphtheria, Tetanus, Pertussis (DTaP): New requirement for 11 year old students.

Booster dose of Tdap required at age 11. 

Students with birthdates from 9/01/1997 to 8/31/1998 will be required to have the Tdap booster. 

If the student is over the age of 12 a Tdap booster will only be required if it has been 10 years since the last DTaP/DPT/Td.

If the pertussis component of the vaccine is contraindicated, the provider should complete the KCI Medical Exemption Form.  If a pertussis outbreak occurs in the school, the student would be excluded from school in accordance with state guidelines.

7th GRADE THROUGH HIGH SCHOOL STUDENTS will need the Tdap vaccine if it has been 10 years since the last dose of DTaP/DPT/Td.  If the last dose was given between January 1999 and May 2000 a Tdap will be due for the next school year.

Hepatitis B:    

THREE DOSES REQUIRED FOR ALL STUDENTS THROUGH 9th GRADE

                

Varicella (chickenpox):    

  ONE DOSE REQUIRED FOR ALL STUDENTS THROUGH 9TH GRADE.

This is not required if proof of chickenpox in the past. This requires documentation by a physician if not previously documented on immunization records with parent verification. Parent verification CAN NOT be accepted next school year.

Two doses of varicella required for Kindergarten, unless the student has had chickenpox and documentation by a licensed physician.

NEW EARLY CHILDHOOD PROGRAM REQUIREMENTS:

Hepatitis A: two doses required for children less than 5 years of age.

Haemophilus influenzae type b (Hib): three doses required for children less that 5 yrs. of age. Total doses dependent on the age of the child when doses given.

Pneumococcal conjugate (PCV7): four doses required for children less than 5 years of age.  Total doses dependent on age doses given.

Immunizations need to be completed prior to the first day of school.
